Guest Waivers and Orientation Information

Guests of Elim Park Place residents are welcome to use the Wellness Center. The following requirements are necessary.

A signed Waiver of Liability must be on file and initialed by a Wellness Staff member prior to using the Center. The Witness can be a family member, a wellness staff member or friend. Waivers for children must be signed by the parent. Grandparent signatures are not acceptable.
An Orientation Information sheet must be read prior to using the Wellness Center. Guests must sign the bottom of the Waiver stating that they have read the Information.
Residents must accompany guests while they use the Center.
